How Do I Know If I Have Acute Back Pain (Hexenschuss)?

Acute lumbago is usually easy to recognise: sudden, intense pain in the lower back, often following a twisting movement, a lift in an awkward position, or even a change in pressure (like a sneeze or straining). The pain typically stays localised in the lower back and does not radiate into the leg. Movement can be severely restricted — standing up from sitting or straightening from a bent position feels nearly impossible.

If the pain also radiates into the buttock or leg, if numbness or tingling occurs, or if symptoms persist beyond three days, a sciatic nerve irritation or disc problem may be involved — both conditions we also treat and investigate where appropriate.

What To Do Immediately — First Aid for Acute Back Pain

The instinct of most people is complete rest or deep heating for extended time. This is understandable but only partially right. Full bed rest typically worsens acute back pain — the muscles cramp further and the joint blockage becomes more fixed. Long periods of heat (more than 20 minutes) can actually increase inflammation, and when the heat is removed, the pain often worsens again.

What helps in the short term:

Moderate movement is better than laying still. Short, careful walks keep the muscles loose and promote circulation. Heat relaxes cramped musculature — a short-duration with a heat pad or warm bath can bring relief in the first hours. Anti-inflammatory painkillers such as ibuprofen can help bridge the worst of the acute phase — but they do not address the underlying cause, and they are not a long-term solution.

What you should not do: aggressive stretching during the acute phase, lifting heavy objects, staying locked in a fixed posture for long periods, sleeping on a heating pad or patch, and — most importantly — waiting and hoping it resolves on its own. It is safest to have it addressed by a professional.

What Causes Acute Back Pain?

Acute lumbago occurs when the small facet joints of the lumbar spine lock up, or when the surrounding back muscles go into a protective spasm. This blockage is mechanical — in most cases it involves no serious tissue damage, even though the pain would suggest otherwise.

 

Common triggers include sudden twisting movements, lifting in an awkward position, cold or draught exposure, chronic postural strain from prolonged sitting, or simply an accumulation of small stresses that eventually tip the balance. Even something as simple as sleeping in the wrong position or in a new bed can trigger lumbago.

Yes, the same mechanism can affect the cervical spine. Sudden, severe neck pain with strongly restricted head rotation — often appearing after waking up — is caused by joint blockage in the neck vertebrae and surrounding muscle spasm. It can also occur with or without a headache, jaw tension, and even shoulder pain.

What should you not do with acute back pain? Complete bed rest, aggressive stretching during the acute phase, lifting heavy objects, and remaining in a fixed posture for extended periods. Cold can worsen muscle tension in the acute phase — heat is generally more helpful. However, extended periods of deep heating can also increase local inflammation (longer than 20 minutes) and cause rebound pain when it is removed. Call your provider if you are unsure, anxious, or pain becomes pulsating or electric. 

Do home remedies help with acute back pain? Short-term heat (20 minutes) with a warm water bottle or warm bath can relax the musculature and provide some relief. Gentle movement is better than complete rest. These measures help bridge the gap until treatment — but they rarely resolve the underlying joint blockage.

Is stretching good for acute back pain? Aggressive stretching is not recommended during the acute phase. Gentle mobilisation — short walks, careful knee movements while lying down — is preferable to forced stretching. Once the acute phase has passed, targeted stretching and strengthening exercises are valuable for preventing recurrence.

 

Is acute back pain dangerous? In most cases, no. Hexenschuss is painful but not dangerous — it is a mechanical blockage without structural damage. However, if pain radiates into the leg, numbness or weakness in the leg occurs, or bladder and bowel function is affected, seek medical attention immediately. Pain that pulses like a heartbeat, causes nausea or occurs with a fever is not normal and should be addressed urgently.
